Delta Care HMO identification cards will arrive in 2-3 weeks from the date that you complete your enrollments in Alight Worklife (formerly "SmartBen"). Cards are not issued by the Delta Dental (PPO) carriers. Please provide your Delta Dental (PPO) practitioners with your social security and your group number (07128). Alternatively, you can create an online account at https://www1.deltadentalins.com/  and print a generic ID card.

  • On the left-hand side of the page, complete the information under the “Find a Dentist” box
  • Choose Delta Dental PPO under the “Select Network” drop down list and click “Find a Dentist”
Health Maintenance Organization (HMO) DeltaCareUSA
  • Visit www.deltadentalins.com
  • On the left-hand side of the page, complete the information under the “Find a Dentist” box
  • Choose DeltaCare USA under the “Select Network” drop down list and click “Find a Dentist”

The district provides a set annual contribution towards your benefits based on your medical coverage tier (Waive, Employee Only, Employee +1, Employee +2 or more).  The district contribution amount is applied towards your medical, dental, and vision coverage.  Please refer to the "Health Options and Costs" sheet (available on the Benefits tab) specific to your employee group for more detailed information.
